Job Title: 100% Remote LPN ( Care Management Coordinator )
Location: 100% Remote
Duration: 6 Months (with possible extensions)
Pay: $18/hr (W-2)
Position Overview:
The Care Management Coordinator (CMC) plays a vital role in supporting pediatric shift care services. This position combines administrative responsibilities, clinical review, and communication with various healthcare stakeholders. The ideal candidate is organized, detail-oriented, and has a solid understanding of medical processes and terminology.
Key Responsibilities:
Primary Support to Clinical Staff: Serve as the main point of contact and provide administrative and operational support to the clinical team.
Liaison Role: Coordinate between home health agencies, provider offices, case managers, and other care team members.
Medical Record Review: Review pediatric medical records to understand members conditions and care needs.
Summarization for Medical Director: Condense complex medical information into clear, concise summaries for review.
Authorization Management: Manage and process authorizations for pediatric shift care services.
Time Management & Documentation: Adhere to strict timelines and maintain accurate, detailed documentation.
Required Skills & Qualifications:
High School Diploma or GED (required)
Active LPN License (required)
U.S. Citizens only
Proficiency in computer applications, especially Microsoft Excel
Familiarity with electronic medical records (EMR) systems
Strong communication skills for interacting with providers and healthcare partners
Basic understanding of medical terminology
Experience in medical , utilization , or authorization management preferred
