Job Title: RN – Case Manager / Utilization Review / Clinical Documentation Improvement (CDI)

Location: Syracuse, NY

Contract Duration: 13 Weeks

Pay: $56–$65/hour

License: Registered Nurse (RN)

Job Summary:
"Navitas Healthcare, LLC" is seeking a skilled RN Case Manager with experience in Utilization Review and Clinical Documentation Improvement (CDI) to ensure patient care meets clinical, regulatory, and utilization standards. This role involves evaluating medical records, coordinating care, supporting accurate clinical documentation, and collaborating with multidisciplinary teams to optimize patient outcomes.

Key Responsibilities:

• Conduct utilization review to ensure care is medically necessary and appropriate

• Perform clinical documentation improvement by reviewing and clarifying physician documentation

• Monitor patient care plans and coordinate with interdisciplinary teams

• Facilitate transitions of care, including hospital admissions, transfers, and discharges, ensuring compliance with regulatory standards

• Provide guidance to staff and providers on care management and utilization

• Accurately document findings, recommendations, and interventions in EPIC or other EMR systems

• Serve as a resource on clinical, regulatory, and documentation requirements

• Communicate effectively with physicians, nurses, and ancillary staff to ensure optimal patient care

Qualifications:

• Current New York State RN license

• BLS certification (current and recertifiable)

• Minimum 1.5 years of recent acute care experience in a hospital setting (within past 2 years)

• Experience in hospitals of equal or greater size or Level I/II trauma centers preferred for travel candidates

• Experience with EPIC Electronic Medical Records preferred

• Strong interpersonal and communication skills
