Job Description

Job Responsibilities:

Perform telephonic comprehensive medication reviews, which include but is not limited to ensuring a complete medication reconciliation is completed and performing clinical assessments on applicable conditions for each targeted member.
Identify, document, and facilitate resolution of identified medication related problems.
Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy.
Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems.
Build and maintain positive relationships with internal and external customers including physicians and nurses

Skills:

Applicants must reside and have an active pharmacist license in either HI, TX, IL, FL, or PA
Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.
Basic computer skills and proficiency with Microsoft Office products.
Strong organizational skills, interpersonal skills, and detail orientated.
Experience maintaining focus and meeting goals in a work from home environment.
Previous call center experience

Education/Experience:

Bachelor’s Degree in Pharmacy or PharmD from an accredited school of Pharmacy
